off meds maybe sometimes on but heart races really hard and fast is this common with adhd he works himself up because all he does is worry about everything, spiders, tornados, dying etc etc

The first problem is you have to be consistent with your medication - NOT on sometimes off sometimes.  The heart racing can be a side effect of the medication, but it is always best to check it out with a cardiologist also.  I had this problem with my 13-year old sone when he was 7 and we went thru all the non-invasive, no pain test to be sure his heart was fine.  It is.  We changed the medication, which often has to be done, and it subsided.  Occassionally, it will race, but along with ADHD comes anxiety so there is a lot going on with the hormones in the brain.  Worrying about everything is OCD, another co-existing condition to ADHD.  OCD can also be debilitating if the fears interfere with your daily life as it seems to above.  Are you seeing a psychiatrist?  If not - do and be sure it is someone that has complete understanding of how the hormones work with and ADHD brain and be sure to be completely honest with the Dr.  It is the only way they can help.  But DON’T play with your medication.  It makes matters much worse.  Find one that works for you and stay with it.  Unfortunately, many times it is trial and error.  I don’t know how old you are, so it could be harder if you are younger, but it will be worth it in the end.
